If Heather had lived and "Poltergeist III" had made more money, yes, there surely would have been a fourth. The title of the script during filming of the third movie was even "We're Back...Poltergeist Continues." Notice it wasn't "Poltergeist III: The Final Chapter" (although apparently this title was considered before release and was used in Austrailia). In addition, the studio ended III with the bolts of lightning hitting the high rise and Kane's evil laughter-a clear opening for another sequel (at least at that time). Years later, director Gary Sherman would claim that he intended "Poltergeist III" to be the end of the trilogy. Near the end of shooting on P3, Heather O'Rourke was quoted as saying that if there was a "Poltergeist IV," she hoped it would be the last, since she was starting to get tired of playing the role of Carol Anne. So, perhaps the studio went back and forth about whether to do another sequel or not. At the very least, Sherman's shooting script appears to have drawn to a conclusion the original conflict between Carol Anne and Kane (it originally ended without any lightning bolts or evil laughter).
