  • Continuity: Malcolm's hair falls to the side when he does his "magic trick" for Cole in the library. When the camera cuts back to Malcolm, his hair is fixed.

  • Continuity: When Cole steps away from the chair because Malcolm can't guess his thoughts: in close-up he uses the other foot to step back.

  • Continuity: The first time Malcolm meets with Cole, right before they leave, Malcolm's arm is on the back of the pew from Cole's point of view, but from Malcolm's point of view, it is down. Right after Cole grabs the statue of Jesus, he puts his arm up again.

  • Errors in geography: Malcolm follows Cole from his house in South Philadelphia, to the church, which is assumed to be within walking distance. However, the church is St. Augustine's, located a good forty (or even closer to fifty) blocks or so from South Philadelphia, a long way for a child to run.

  • Crew or equipment visible: During the dinner scene, mic cord visible through Malcolm's shirt.

  • Continuity: When Malcolm finishes the magic trick with the penny and asks, "Do you really think so?" he closes his hand around the penny. In the next shot taken from his back, his hand is still open.

  • Continuity: When Cole asks "Will I see you again?" in the church, Malcolm's arm jumps back and forth between the pew and his side between shots.

  • Continuity: Cole has a light blond patch of hair on the back of his head. While it is normally on the right side, when he sits next to Kyra's sister on the swing it is suddenly on the left side.

  • Continuity: When Malcolm walks into Cole's hospital room, Cole's feet are outside the covers. In the next shot one foot is under the covers.

  • Continuity: When Cole is walking with Malcolm through the streets of Philly to go see the German man, the streets and sidewalks are covered with brown fall leaves, but the trees along the street are all green as if it is spring time.

  • Revealing mistakes: When Cole is yelling "Stuttering Stanley" at his teacher, lingual braces (they are fixed to the back of teeth rather than the front) can clearly be seen on Cole's teeth. Given their expense vs. regular braces, it is highly unlikely that Cole's mom would opt for this type of orthodontic appliance when she is working two jobs to make ends meet.

  • Crew or equipment visible: During a class in school, you can see microphones in the roof.

  • Continuity: When Malcolm and Cole meet in the church for the last time, a patch of hair is sticking up from the right-hand side of Cole's head. The hair becomes less frisky between shots until it ends up well combed.

  • Continuity: When Cole is in the bed about to say "I see dead people" the pink blanket continually rearranges itself in each shot.

  • Anachronisms: When Malcolm is waiting to meet Cole for the first time, we get a shot of the paper he's been writing, and a good look at certain circled words that compare Cole's case to an earlier case. Later, after their meeting, we get a chance to read that paper, and it becomes obvious that the words he had already written were actually a description of the event yet to occur: his first meeting with Cole.

  • Continuity: When Malcolm is listening to the Vincent Grey tape, in the close-ups of the cassette the amount of tape on the spools vary greatly between each shot.

  • Continuity: The tape that is labeled "Vincent Grey" in the first shot has the label missing in the next shot.

  • Continuity: During the "I See Dead People" scene, Cole is facing Malcolm and a tear runs down his face. When Cole turns away, his face is dry with no trace of a tear.

  • Continuity: At the end of the film Malcolm Crowe comes down the stairs to the living room. The camera is at a low angle looking up from the living room floor toward the vestibule area at the foot of the stairs. There is a leather covered sofa and two overstuffed leather chairs crammed side by side to fit into the space. Crowe's wife is slouched in one of the chairs. The camera changes to above, the wife falls asleep and drops Crowe's ring, which now rolls over open floor boards. Where did all the furniture go?

  • Continuity: When Cole sees the ghost boy with a gun shot wound to the head, although its seen on the back of his head, in an earlier shot, it wasn't there.

  • Continuity: During the meal scene between Cole and his mother, Cole throws on of his gloves at the TV set. In the next shot he still has two gloves in his hands to lay on the kitchen table.

  • Revealing mistakes: When Cole and Malcolm are going up the stairs to Kyra's bedroom, the banister wobbles.

  • Revealing mistakes: You can see Mischa Barton under the bed getting ready to grab Cole's ankle in her room.

  • Factual errors: When Malcolm (Bruce Willis) is watching his wife in the shower, he notices the Zoloft prescription for depression. The label clearly states, "Take with food or milk." However, no such warning or contraindication against Zoloft or it's generic, Sertraline, exists.
