- Audio/visual unsynchronized: When the robber with the ski mask points his revolver at Captain Spaulding for the last time you hear the sound of the hammer being cocked, yet in subsequent shots the hammer is uncocked.
- Continuity: During Bill's torture his sweater vest has a different pattern.
- Continuity: Jerry's right hand jumps on and off his mug between shots.
- Crew or equipment visible: In the graveyard scene with Mary, the shadow of the camera and cameraman can be seen as she spins around.
- Continuity: When Mother Firefly pulls the pistol on Lt. Wydell the revolver seems very large, and the hammer has been cocked back. When she fires, the hammer is already down and does not drop. The gun also appears to be a different, smaller weapon when the scene cuts to Wydell being struck and killed.
- Continuity: The TV announcer pronounces Dr. Wolfenstein's name "Wolfen-STEEN". Dr. Wolfenstein pronounces it himself "Wolfen-STINE".

Goofs below here contain information that may give away important plot points. You may not want to read any further if you've not already seen this title.
- Revealing mistakes: SPOILER: When Bill is shown getting his right arm cut off, the ax hits the obviously fake arm and turns sideways. It does not go all the way through, but the ax wielder is shown immediately afterwards holding the neatly severed limb after only the initial swing.
