  • Factual errors: The prisoner is described as carrying an "Axis IV Dissociative Disorder." In psychiatric diagnosis, psychiatric disorders fall under Axis I, personality disorders and mental retardation fall under Axis II, medical disorders fall under Axis III, a general summary of psychosocial stressors (eg. health problems, money problems, or family problems, and rated as None, Mild, Moderate, or Severe) fall under Axis IV, and the patient's Global Assessment of Function score (GAF: 0 to 100) falls under Axis V. Dissociative Disorder is an Axis I diagnosis.

  • Revealing mistakes: The newspapers shown during the opening credits are quite accurate, revealing additional information about the killer's background. In one case, though, two columns show exactly the same text.

  • Continuity: When Alice is first hit by the car, the car pulls her down, and you can see her start to go under the car. When Alice is hit from Ed's P.O.V., Alice hits the hood of the car, rolls over the top, then slides off the trunk onto the road.

  • Crew or equipment visible: When the two convicts take control of the prisoner transport car, the stunt driver (dressed in a light blue top, unlike the prison escort driver) can be clearly seen bringing the skidding car to a controlled stop.

  • Revealing mistakes: When Paris backs her car into the power pole, the gearshift can be clearly seen in the "Park" position.

  • Revealing mistakes: When Ginny is in the locked bathroom, and Lou is being killed outside of the door, the door is shaken and the slide lock falls apart, yet the door remains locked.

  • Revealing mistakes: When Caroline is talking on the phone while underway in the limo, the digital speedometer clearly reads 0 mph.

  • Revealing mistakes: When Ed is driving just before hitting Alice, the camera is looking over his shoulder. You can clearly see that the digital speedometer reads 0.

  • Continuity: When they are tying up the "hotel manager" the digital clock reads 12:20 throughout the scene.

  • Errors in geography: At the end of the movie they are supposed to be in Frostproof, Fl, but as the camera pans back there are mountains in the background of the orange grove. The shot looks a whole lot more like California, and not Florida.

  • Errors made by characters (possibly deliberate errors by the filmmakers): In the second scene when Larry is watching Wheel of Fortune, he yells at the contestant to buy a vowel, and then tells him to ask for an "N". But just before then when they showed the TV screen, N was already guessed and on the board.

  • Incorrectly regarded as goofs: SPOILER: Various inconsistencies, improbabilities and impossibilities in the motel scenes are not actual errors. This is all happening inside Malcolm's head, so the rules and geography of the real world don't necessarily apply.

  • Continuity: SPOILER: In the flashback showing how Rhodes killed his police escort, the policeman's foot is sticking out of the trunk. Rhodes then slams down the lid without moving the leg out of the way.

  • Plot holes: SPOILER: The police car that brought the convicts was hot-wired. Upon seeing the flashback of how Rhodes obtained the car, there is no logical reason, even in Malcolm's head, why the car would need to be hot-wired, as the keys were still in the ignition.
