- Continuity: In Dr. Glass' office, Catherine Tramell drops her cigarette on the floor by the door and steps on it to extinguish it. A few moments later when she leaves, the cigarette is gone.
- Revealing mistakes: As Dr. Michael Glass is investigating the article entitled "Detective X" by Adam Towers about Roy Washburn, under the sub-headline of Witness Intimidation the article reads "A perfectly good movie script could be made from the information I gleaned in the sewers and alleys and fire escapes of the seedier parts of town." When he reads on to the next sub-headline of the article "Falsifying Evidence" the exact same passage has been repeated: "A perfectly good movie script could be made from the information I gleaned in the sewers and alleys and fire escapes of the seedier parts of town." Additionally, the sentence directly preceding the second sub-headline is incomplete. It reads "Our current political leader" (No punctuation)
- Continuity: At the end of the movie, there is a scene where Dr. Glass is sitting in his wheelchair in the outdoor hallway of the Crazy House. Catherine walks up to him and she places her book on a table beside a chair. The table and chair were not present in the previous shot.
- Continuity: When the detective picks up Michael to go to a murder scene, we can see that Michael has not shaven for at least a day. When he steps in the car you can see right away that he is clean shaven.
