• Crew or equipment visible: In the opening scene right before Audrey walks into the house, there's a table lined with pictures and as the camera is passing by you can get a quick second-long glimpse of the camera as it passes.
  
  
• Continuity: When Audrey enters Eason's dark room, she turns on the lights, but seconds later the light is red again.
  
  
• Continuity: When Trish enters Jake's room, the floor in front of him is empty, and the camera turns to face Trish. When it's put back on Jake there is some kind of sport equipment on the floor.
  
  

• Factual errors: When Aubrey goes into the dark room and picks up the picture, the picture should be black. It was in the developer last. It would have had to go into the stop bath to stop developing, and then the fixer to not go black when it was exposed to real light.
  
  
• Continuity: When the girl enters the phone booth the door is open, but when she turns around it is shut.
  
  
• Crew or equipment visible: When Vanessa at the office mobile phone calls by Kayako, she fears from voice of Kayako. You can see the ceiling move two microphones.
  
  
• Revealing mistakes: When reading the diary, the characters read the book left to right, like a book written in English. Japanese, when written in tategaki format (top to bottom) or yokogaki (left to right), still is opened and paged right to left. However, the characters correctly read the printed book in the same scene.
  
  

• Continuity: SPOILER: When Karen is seen on the floor after she falls from the hospital roof there is a considerable amount of blood behind her flowing from her head. Eason then sees Kayako on Karen and there is no blood behind Karen at all.
  
  
• Continuity: SPOILER: When Trish comes into the kitchen in the opening scene, the clock on the wall is 8:31. In the close-up of her hitting her husband, the long hand of the clock has moved, making it 8:35, then as she sits down, the clock is once more 8:31.
  
  
• Continuity: SPOILER: When Eason is talking to Aubrey outside the hospital, he looks up to see Karen falling from the roof on his left, but when she hits the ground he turns to look on his right.
