- Errors in geography: The movie plays fast and loose with New Orleans geography throughout. For example, during the "chase", Carlin is driving towards I-10 (on the east bank). Seconds later, he is driving through the Harvey Tunnel, several miles away on the west bank. Then he's back on the east bank, heading west on I-10. New Orleans geography adds to the confusion; Carlin somehow makes a U-turn on the Crescent City Connection, which goes eastward towards the west bank and westward towards the east bank.
- Factual errors: When the ferry explodes, vehicles are thrown into the river. The Algiers-Canal ferry is pedestrians only on Fat Tuesday. Also, for the same reason, the car with the explosives wouldn't have been allowed on board.
- Continuity: When Doug Carlin is walking into the compound holding the helmet in one hand and a gun in the other, there is a shot which is mirrored; the helmet and wristwatch are on the right and the gun appears to in his left hand.
- Revealing mistakes: During the car chase, when the red Pontiac is falling to the street after flipping over, the roll cage inside the car is briefly visible through the windshield.
- Factual errors: The Navy Uniforms are incorrect for Fat Tuesday. The Officers and Sailors are wearing Summer Whites while the prescribed winter uniform in the Continental U.S. for February is either Service Dress Blue or Winter/Working Blue. There are also personnel wearing Khakis. While authorized for wear in winter the Khaki uniform usually is worn with a black pullover sweater or black jacket in the winter.
- Continuity: When Doug is takes off his shirt just before entering into the "teleporting" machine, in the next shot you can see him taking the shirt off again.
- Continuity: During Claire's autopsy, just before Doug takes her photo with the Polaroid, her mouth is completely closed. In the next shot and in the photo, her mouth is slightly open with her teeth showing.
- Continuity: At the beginning of the movie, when Doug parks his car at the pier, a clear shot of the ATF sticker and the month/year sticker is made. On the month/year sticker appears 07 for the month and 06 for the year on a white and blue background respectively. At the end of the movie, when he is leaving with Claire on the same car, the month/year sticker has changed now to 08 for the month and 06 for the year and with white and gray background respectively.
- Continuity: When Doug reaches to the pier at the beginning of the movie, the camera makes a shot that shows the ATF sticker and the month/year sticker located at Doug's car front window. The ATF sticker is below the month/year sticker, but at the end of the movie, the ATF sticker moved a couple of inches to the right side of the month/year sticker.
- Continuity: When Doug is investigating Claire's house for the first time, he enters her living room and closes the door behind him. He looks at the giant jazzy painting that is all over the wall and the door. Two shots later the door behind him is suddenly open again.
- Errors in geography: The compound is located in Stephensville, LA (just North of Morgan City), about 90 miles away from New Orleans, not just an exit in New Orleans, as the movie suggests.
- Revealing mistakes: During Claire Kuchever's autopsy, when her head is turned toward the camera, her eyes follow a distant fixed point, and not the rotation of her head. This causes her irises to become centered in her eyes by the end of the head turn. In other shots, her eyes are fixed looking slightly to her left.
- Continuity: Outside the hospital after Doug is revived, the paramedic that Doug confronts initially has a cigarette in his left hand. Seconds later, it switches to his right hand, despite the fact his hands are raised.
- Incorrectly regarded as goofs: In the compound scene a St. Martin Parish Sheriff's car can be seen parked in the background. New Orleans is located in Orleans Parish, over 150 miles away from St. Martin Parish. A St. Martin Parish Sheriff's deputy would not have jurisdiction in Orleans Parish. While this would normally be correct in post Katrina New Orleans there were many officers from many different jurisdictions in state and out of state.
- Revealing mistakes: During the explosion at the Bait Farm after Denzel Washington drives the ambulance into the hut, a large metal beam can be seen flying towards and 'bouncing off' of Claire’s 'body', obviously a dummy.
- Continuity: When Doug Carlin's Hummer is driving up on the bridge after the truck crashes into him during the car chase, the camera shot from within Carlin's car shows him passing a VW Beetle. When the camera switches to an overhead one, we can see that Carlin's Hummer hasn't passed the Beetle yet.
- Revealing mistakes: In the opening sequence you see sailors moving from the left of the screen to the right but all of the rating badges and service stripes are on the incorrect arm. These are suppose to be on the left arm but they appear on the right indicating that the film makers used a mirror image of the shot.
- Revealing mistakes: During Claire Kuchever's autopsy, the coroner asks Doug Carlin to hold her right hand while a photo is taken. The coroner explains that her fingers have been severed. The camera angle changes and you can see that her fingers are curled under.
- Continuity: When Doug gets out of the Hummer outside the compound holding the helmet, his wrist watch is on his left arm. Once he starts walking towards the compound, the watch is now on his right arm. The watch appears on his left arm again later in the scene.
- Plot holes: Agent Carlin discovers the evidence of blasting caps washed ashore at 1245; he returns the call to the victim at 1318 (1:18pm). He seems to do quite a lot in those 33 minutes: investigates the surface and underside of the Crescent City Bridge, visits the police department to notify them that it is a crime scene, gets a cup of coffee there (by inquiring the location of the coffee pot), drives back to the ATF office, and attempts to reach his partner.
- Continuity: The bomber arrived at the ferry dock at 0440 which means Carlin's chase could not have started before 1040. During Agent Carlin's pursuit of the bomber, we get a glimpse of Pryzwarra's watch when he is picking up the phone that shows 5:45 which could be 0545 (five hours too early) or 1745 (meaning Carlin took seven hours to get to the Crescent City Bridge). When Carlin starts to walk away from the Hummer in daylight, his watch shows 0950 (50 minutes too soon) but when he gets on the other side of the house, the time shows 1135 (more in sync with the timeline).
- Continuity: When Claire starts writing in her diary after taking a shower, she writes immediately below the previous entry. The next shot, which gives us a close-up, shows there are two blank lines between the previous entry and the one she is making.
- Continuity: When Doug saves Claire from the warehouse blast, as they land on the floor, her feet aren't bound together. In the next shot, he removes the tape from her bound feet.
- Continuity: When Doug is angered about knowing the truth about what they were actually doing to view everywhere other than "satellites", he tries to make a point clear about something being present and past by taking a chair and breaking a computer monitor. Yet in later scenes the monitor seems to be working again, unless they replaced a monitor in a matter of minutes.
- Continuity: During the first scenes on the ferry a girl's doll falls into the water. You can see the doll moving towards the camera and briefly hitting it before the shot changes and the doll keeps sinking.
- Continuity: When investigators are searching Claire's house, Doug points out that it's (just) after midnight. At this point in the movie, it's night, but when the investigators are searching Claire's house (same time zone), it is light out.
- Continuity: When the team sees the video tape of Carroll Oerstadt at the ferry (after using the facial recognition software to identify Oerstadt's "camcorder bag"), it is indicated thereon as "Monday", and Dr. Alexander Denny says that the tape is from two nights before the ferry explosion. But the ferry explosion is on Fat Tuesday, so the tape was really from the night before, not two nights before.
- Continuity: During Claire's autopsy, Doug turns her head to her left, to find out that she has a missing earring. In the next shot however, her head is tilted to her right again.
- Continuity: When Agent Larry Minuti, Doug's partner, attempts to apprehend the terrorist on the pier, he is shot through the closed door of the terrorist's car. In the next shot the car door is open, the window is fully intact, and there is no visible sign of the bullets passing through the car door.

Goofs below here contain information that may give away important plot points. You may not want to read any further if you've not already seen this title.
- Plot holes: SPOILER: The bomber calls Claire on Sunday evening, 26 February 2006, at 1946 (7:46pm). Since the feed is four days and six hours in the past, the time that they are seeing this is Friday morning, 3 March 2006 at 0146 (1:46am). The phone log trace shows times of calls on 3 March well into the afternoon and evening, as last as 2010 (8:10pm).
