• Factual errors: Alderney is sometimes referred to as Guernsey in dialogue - Which in real life is part of the Channel Islands (Jersey, Guernsey, Alderney, et al).

  • Audio/visual unsynchronized: In the mission in which you chase the laundromat owner, Niko says "Just slow the hell up!" but the subtitles read "Just slow the fuck up!"

  • Continuity: In the LCPD Database Hossan Ramzy is listed having been arrested in 2007 on a counterfeit purse charge. Yet it was 2008 when he arrived in Liberty City with Niko, went down to Florida and then finally returned to Liberty to sell purses.

  • Audio/visual unsynchronized: In one of the boating activities you go on with Brucie, he tells you to go to the "Statue of Freedom" and the subtitles read "Statue of Happiness", which is what it is actually called in the game.

  • Continuity: In the cutscene for the mission 'Museum Piece', Niko enters through a swinging door. However the behaviour of this door changes throughout the cutscene. When Niko attempts to leave, Ray is able to slam it closed, and at the end of the cutscene the door is able to stay in the one position to allow us a view of Ray after Niko leaves.

  • Factual errors: In the mission "Flatline", a character is on a life support machine, as is stated when you attempt to turn it off. However, the character is well able to speak, something which would be highly unlikely if his condition necessitated the machine.

  • Errors made by characters (possibly deliberate errors by the filmmakers): The police radio dispatcher mispronounces the name of the vehicle manufacturer "Zirconium" as "Zireonium" (note: "Zireonium" can also be the beta name for "Zirconium").

  • Errors in geography: Alderney is an independent state, and so is Liberty City not a part of it. But when you go to Alderney, it's police cars have the LCPD (Liberty City Police Department) decal on the sides. The officers themselves have a different uniform though.

  • Revealing mistakes: If one types in 'Little Jacob' in the Police car database, it will find Clarence Little instead. Jacob can be found by typing in 'Jacob Hughes', however.

  • Factual errors: When traveling down Babbage Drive in Central Alderney, it is a 4-lane road, like a highway, and there is the north and south bound lanes. Like a highway, the north and south bound lanes have two lanes each. When looking at either highway lane, it has the double solid yellow line, which would mean that the northbound and southbound lanes are in fact, a two-way road, and that Babbage drive is in fact, two different roads.

  • Errors made by characters (possibly deliberate errors by the filmmakers): If one has a wanted level whilst driving a Cognoscenti, the Police dispatcher will refer to it as a 'limo'.

  • Continuity: If one attains a 4-star wanted level and then goes to the airport, the wanted level will drop to a 3-star wanted level. But, if the player has no wanted level then goes to the airport, a 4-star wanted level will be given.

  • Errors made by characters (possibly deliberate errors by the filmmakers): One of the Mafia crime families is named the Ancelottis. The Italian pronunciation would be 'AnCHelottis'; despite this, most of the Italian characters mispronounce the name, besides Niko and others.

  • Errors made by characters (possibly deliberate errors by the filmmakers): On Public Liberty Radio's 'The Pacemaker' show, Waylon Mason explains that his mother died during childbirth, but later refers to her as "leaving him in dirty diapers all day", despite the fact she died during his childbirth, which is verified when he and his father home-cremated her.

  • Continuity: If you attain a wanted level of four or more stars, and travel to the airport, your wanted level will decrease to either 4 or 3 stars depending on your wanted level.

  • Errors in geography: Travel to the west side of Ruby Street (West of Middle Park) and drive west until you hit a brick road. You will see two signs on either side of the road saying "Do Not Enter." When you go to the other end of the brick road, there are two signs facing out that say "Do Not Enter" and there aren't any other roads connecting to it. If it was a pedestrian path, there should have been blockades, most likely concrete poles.

  • Continuity: Although never killed in the game, Bulgarin is listed as 'deceased' on the LCPD Database.

  • Continuity: SPOILER: If you choose to kill Trey 'Playboy X' Stewart, his profile on the online LCPD criminal database does not list him as deceased. However, in the Police car's database Stewart is listed as deceased.

  • Miscellaneous: SPOILER: Joe 'Tuna' DiLeo is not listed as deceased in the LCPD Database despite being killed.

  • Revealing mistakes: SPOILER: In the cutscene for Gerry McReary's mission 'I'll Have Her', a sign in the background says "Liberty City Correctional Facility Duty Officer Rotation"; despite Gerry being in the Alderney State Correctional Facility.
