Because of phonetic differences between English and Korean, both "Mother" and "Murder" are spelled the same when translated to Korean characters. The movie title, "Madeo", is a play on this similarity, suggesting both "Mother" and "Murder".

Joon-ho Bong originally conceived the idea for the film long before he made The Host (2006). He wrote his first synopsis in 2004 which was round about the time he met Hye-ja Kim for the first time.

Won Bin's character has a younger fraternal relationship with a character named Jin-tae in this film. Jin-tae is the same name of Bin Won's brother in [Tae Guk Gi: The Brotherhood of War (2004)].
