Continuity 

Early in the beginning when John Wick is getting his car back and is being chased by the goons, the back window of his car gets smashed at one point, and the front window gets a crack in it. Both windows are intact in later scenes.

In the opening scene, a man on a motorcycle is being pursued by John Wick in a car. It stops when the motorcycle and rider crash into the passenger door of the car. John gets out of the car, walks around to the other side and the rider, cycle, and debris are on the ground, but the passenger side of the car is visibly intact.

When Wick walks into the Continental Hotel, his white shirt is still dirty as he has just walked from his house being blown up and burned down. However, after he leaves his dog at the front desk, when he arrives on the rooftop garden to see the manager, he is wearing a clean shirt.

Errors in geography 

When Cassian locates John Wick in New York City, he does so at Lincoln Center, where several of its well-known landmarks are plainly visible. Immediately thereafter, Cassian chases Wick into the subway -- except the station that they're in is the new Santiago Calatrava-designed transit hub at the World Trade Center site. The two locales are roughly 80 blocks apart, but are depicted as being adjacent to each other.

When John is at the Continental Hotel in downtown Manhattan, he goes upstairs to meet Winston on the rooftop garden. That rooftop garden is in midtown at Rockefeller Center. You can see the Citybank Tower just behind it in the background.

Factual errors 

When John Wick is gun-fighting in the catacombs, his shotgun does not actually have a stove pipe malfunction. Instead, the customized Benelli shotgun (customized by Taran Tactical) actually has a single shell carrier near the side of the ejection port. This is to make for a quick one-round reload after the gun runs empty, much like John Wick demonstrated in the film.

At 0:35:57 the Jewish man is saying to John Wick in Yiddish "zayt matzliah". It is translated as "Happy hunting" in the closed caption, but the real translation is "Good luck".

As happens in most movies and TV shows, firearm suppressors are portrayed as "silencers". Suppressors do not muffle a gunshot completely, they simply reduce (suppress) some of the cracking sound created as a bullet departs the barrel of a firearm in excess of the speed of sound. Every person in the station would be panicking as much as they would without the suppressors on the weapons.

The Sommelier describes the AR-15 he gives John Wick as having an 11.5 inch barrel. It does not, as it clearly shows a length longer than 11.5 inches, and is rather a 14.5 inch barrel.

There are several mistakes when Cassian chases John Wick though the subway station.

They both board a NY/NJ Path train, however, the train announcer says it is a "Broad Street bound C train", which it cannot be if it is a NY/NJ Path train. Also, the trains that terminate at Broad Street are the J and Z trains, not the C train.

The NY/NJ Path train only has stops in Manhattan between Christopher through 33rd Streets, and separately, directly to the World Trade Center. NY/NJ Path train does not stop at any of the stops mentioned by the train announcer (Canal Street; Rector Street; Broad Street); and the several stops mentioned as "next stops" do not follow each other: Rector Street does not follow Canal Street; Broad Street does not follow Rector Street.

When the gang is unloading the taxis early in the film, we see a pallet loaded with cash, gold, and the wooden boxes with all the remaining gold on one side of the pallet. With such a narrow pallet jack centered under the pallet, it would be impossible to balance the load as depicted.

Incorrectly regarded as goofs 

When John Wick is in the hall of mirrors, he attacks a gunman by jumping through a mirror from the back side of it. From that position, it could appear that John couldn't see the gunman. However, the mirrors are movable and it's possible that John looked through a gap in the mirrors, saw the gunman, then jumped through.

After John and Cassian finish their drinks at the bar, Cassian pays for the drinks and leaves. John then encounters Ares sitting in a chair. Ares communicates with John through sign language. She has at least one error and that is her very last sign. She intends to sign the phrase "Round three". However, the sign for "three" is not performed by flashing your index finger, middle finger, and ring finger (that is the sign for the letter "W"). The sign for the number 3 is shown with your thumb, index and middle fingers.

She was not signing "Round Three" but was rather signing John's name (or more accurately his initials "JW" which is very common in sign language when both parties know who you are talking about in order to shorten the number of character signed) with the pinkie making a swirl in the air for "J" and the index finger, middle finger, and ring finger for "W". This corresponds perfectly with the on-screen sub-titles.

John's Benelli M4 has a seven shell mag tube, meaning a maximum eight shots before needing to reload (seven in the tube and one in the chamber). However, John fires eleven shots without reloading when he first picks up the shotgun in the catacombs. You can have a custom ten shell mag tube made for a Benelli M4, but it would be visibly longer than the barrel. However, John's Benelli M4 probably came with a nine round magazine tube, of which case an 11th shot can be ghost loaded into the receiver.

In the first "John Wick" film, Miss Perkins is shot and killed for trying to quietly assassinate Wick in his room at the Continental. But in this film, Wick's punishment for breaking the Continental's "No business on the premises," rule is simply denial of his access to the Continental, and he is given a one-hour reprieve, despite having broken that rule in a more blatant manner by executing Santino in the Continental's bar in plain view of everyone, including Winston. However, due to Winston's friendship with John (he has been shown to be willing to bend the rules), he obviously found middle ground with the High Table and chose to have someone else carry out the job.

At the start of the film, the gold coins are seen to be smuggled into the country with drugs and money. Nothing special, except we later see the manufacturer of the coins at the Continental with Winston, as Winston inspects them shortly before Wick shows up. If the maker of the coins is already in the country, why are they smuggled in??? On top of that, given how powerful the underworld is in the franchise, they could have just had them made in the US. It seems a bit round-about, as well as extremely wasteful and a redundant way, to get something into a place where they already have immense power and control, however, the coins were shown to be valid all over the world. The ones shown in the beginning weren't being smuggled into the country they were just being distributed to different criminal enterprises. It should also be noted that there could be more manufacturers of those coins and Winston was meeting with just one.

Revealing mistakes 

After the fight with Cassian in Rome, John's suit is relatively untouched when he's sitting at the bar, and in other scenes, the back is untouched too. He was shot multiple times whilst fleeing, and as the tailor explained and demonstrated, the body armor is inside the layers of the suit. When the tailor's assistant shoots at the dummies to demonstrate the armor, the cloth is ripped by each shot as the bullet tears the cloth and is stopped by the armor inside. John's suit should be thoroughly shredded by the time he gets back to the hotel.

When John gets into the Mustang early in the movie, you can see that the rear-view mirror is intact. However as he starts driving it around, it is no longer there.

In the fight scene when John Wick boards from the WTC Station on a Path Train, it goes to Exchange Place. The next station clearly it goes to shows Exchange Place station again, although the audio is heard to say Rector Street. After Exchange place, in reality the next station should be Grove Street. Again the announcement is wrong.

When John is driving his Mustang out of the chop-shop where it was stored, he backs up into a bad guy. During the side view of the rear of the car hitting the bad guy, you see the bad guy rolling along the smooth top of the trunk, but once he's over that part, all other angles show a spoiler on the rear hatch.

The shootout in the catacombs culminates in John utilizing a Benelli M4 shotgun equipped with a MatchSaverzTM. This device is a single-shell caddy attached to the forearm of the weapon just in front of the ejection port. It allows the shooter to "chamber feed" a single shell into the gun once it has been shot completely empty and achieved bolt lock back. John is seen utilizing the Match Saver twice in the same action sequence despite the fact that it holds only one shell and is never reloaded.

Errors in geography 

After Wick's house blows up, he is shown walking from there to the Continental in Downtown Manhattan. Besides being potentially a great distance (as where he is shown living is nowhere the close in suburbs along the New Jersey Palisades directly across the Hudson RIver from NYC) he is shown arriving into Manhattan on the Manhattan Bridge.

The only bridge leading from New Jersey to Manhattan is the George Washington Bridge which doesn't look like Manhattan Bridge. And besides the Manhattan Bridge spans the East River between Manhattan and Brooklyn, meaning that he would have had to walk AROUND the perimeter of Manhattan to cross it.

Revealing mistakes 

When John shoots Gianna in the head with his pistol after she has slit her wrists in the hot tub, her head does not move at all. Ripples are evident on the surface of the water, but her head remains perfectly still. This despite being shot by a medium caliber pistol from just inches away.
